This tag was created for those that have medical conditions and need to wear a form of medical id in the event of an emergency. The tag is blank so that the condition can be stamped on it as well as a favorite scripture verse. Please see necklace and bracelet selections to complete you purchase! "Type 2 Diabetic" is just and example, the tag can be customized to reflect most conditions. Stamping can only be done on the front of the tag. This Medical ID Blank measures 1 inch long by 3/4 inch wide.

The 2nd line will be the vertical line (maximum of 8 characters).
NO SYMBOLS OTHER THAN A DOT. We will stamp any date in a two digit format. Please only place letters/numbers in the area provided. DO NOT put spaces, /, *, &, or any other symbols in this area. A period may be used to separate names only. Enter dates in a two digit format with no spacing or separation of month, day, and year, we will separate using a dot.
